E-Statement Enrollment and Agreement
E-Statement Disclosure Agreement
You must consent to electronic delivery of your statements in order to enroll in our E-Statement services. If you do not want to receive your statements electronically, you should check deny at the bottom of the page. If after reading this Disclosure, you wish to consent to electronic delivery of your statement and enroll in our E-Statement services, please select “I agree” below.
Agreement: Consenting to this disclosure confirms your agreement to be bound by all disclosures and agreements and acknowledges your receipt and understanding of this agreement. By entering into this agreement, you understand that WesTex Community Credit Union will no longer be providing you with a monthly paper statement. You will receive an e-mail notification that your statement(s) are available. You also agree to receive any federally required notices, disclosures and forms electronically. If you change your e-mail address, you will notify us of the change. You will also notify us of any changes in your physical address or telephone number(s).
System Requirements: In order to participate in the E-Statement services and receive and retain copies of your statements electronically you will need to obtain access to the following;
· A computer and modem or other device capable of accessing the internet.
· An active Internet Service Provider (ISP)
· A web browser that supports 128-bit SSL encryption
· The ability to download or print agreements and disclosures
· Adobe Acrobat Reader software (available at no charge at www.adobe.com)
Rights and Options: You are not required to enroll in the E-Statement services program. You have the right to terminate your E-Statement services and receive paper statements at any time upon delivery of written notice to WesTex Community Credit Union. If you cancel receipt of E-Statements and wish to resume receiving E-Statements in the future, it will be necessary for you to complete the sign up process again.
Copies of Paper Statements: You have the right to request a paper copy of your statement at any time by contacting WesTex Community Credit Union. The credit union may assess a charge for this service.
Error Resolution: You understand the importance of your role in preventing misuse of your account. You agree to promptly examine your statement and notify us immediately of any errors. You must notify us no later than 60 days after we sent the first statement on which the problem occurs.
Acceptance: By selecting “I Agree” below, you acknowledge that you understand the terms of this disclosure.
